About
The Functional Neurological Disorder (FND) Care Advisor (FNDCA) role aims to support patients with FND in southwest London and Surrey. This service acts as a central contact point for patients and healthcare professionals to provide coordinated and personalised care. It involves direct patient interactions, which can be remote, in-person, or home visits, and helps patients develop self-management skills through personalised care plans created by a multidisciplinary team.
Roles and responsibilities
- Support development of emergency care plans
- Inpatient in-reach sessions for complex admissions
- Link patient care across multiple services
- Promote self-management skills
- Improve experience and clinical outcomes of people with FND
- Facilitate specialist management plans
- Deliver training to clinical teams on FND
Referral guidelines
- Documented & communicated diagnosis by a consultant neurologist
- New referrals open to all HCPs
- >18 years (16-18 if under a FND Group neurologist)
- GP and consultant in SWL or Surrey
- FND main/ joint main concern
- Include NHS number, name and contact details
- Document reason for referral
- People with pre-existing diagnosis can be referred but may need consultant review
